Fall Prevention

Exercise and teaching designed to reduce the risk of falls.

Falls are one of the more serious medical events for seniors.  A study published in the medical journal JAMA found that for people over 75, the rate of mortality from falls more than doubled from 2000 to 2016.  Many falls are also avoidable with the right training and therapy. Congestive Heart Failure and HTN Education

Our goal is to provide teaching and training on these conditions and this amazing human organ.

CHF, HTN, ejection fractions, mitral valves, CABG, Beta Blockers, Angiograms....the heart and the medical terminology  about the heart is all very confusing unless you have years of specialty education.  At Choice Homecare we believe it is important to understand your heart health, the medications, and the medical terms used around you.  We have developed a specialty teaching program to help our patients.  In addition we provide remote patient monitoring so that our patients can receive daily oversight of their condition.

Medication management is a strategy for engaging with patients and caregivers to create a complete and accurate medication list using the brown bag method. A complete and accurate medication list is the foundation for addressing medication reconciliation and medication management issues. These tools will also help to identify patient behaviors that may be putting patients at risk for an adverse drug event, such as overdosing, underdosing, or missing medications, or other important contextual factors limiting adherence.

Occupational therapy practitioners help people with low vision function at the highest possible level by preventing accidents and injury (e.g., improving lighting), teaching new skills (e.g., eccentric viewing, visual tracking), modifying the task or environment (e.g., recommending magnifiers), and promoting a healthy lifestyle (e.g., ensuring they can participate in their daily activities).

Flu Treatment and Support

The Flu virus can be very difficult to deal with at any age.  Our program is specifically designed to help persons over the age of 65 manage the illness without further complications.  Falls and dehydration can lead to hospitalization.  Choice combines a high frequency of visits and specific disciplines along with the coordination of care with your physicians office to help promote a  safe recovery.
